Changes to our Phlebotomy Service at Farnham Road Practice
Farnham Road Practice is changing how we utilise our Health Care Assistants. With immediate effect we will no longer be offering routine blood tests to patients other than those listed below.
Patients with poor mobility who successfully come to the surgery but for whom a journey to the hospital is a significant undertaking
Patients needing frequent blood tests to closely monitor their medication (e.g. those on Methotrexate, Azathioprine and similar medications)
Patients needing monitoring of their condition in response to changes in medication (e.g. ACE inhibitors)
Patients needing urgent blood tests who have just seen a clinician.
We are changing the service we currently offer as we hope that patients needing regular blood tests, and those patients needing e.g. BP monitoring, can get an appointment more easily with the HCAs.
If are asked to have a routine blood test carried out this can be done at:

SUMMARY CARE RECORDS
In line with NHS guidelines the Practice now share some of your medical information so that other health care professionals can access them to provide you with the best medical care. This is called a Summary Care record (SCR). The SCR will contain key information about the medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had in the past.
If you have an accident or fall ill, the people caring for you in places like accident and emergency departments and GP out of hours services will be better equipped to treat you if they have this information. Your SCR will be available to authorised healthcare staff whenever and wherever you need treatment in England. Health care professionals will have to ask your permission before they look at your SCR.
Please click on the link below and return the form to the Practice to enable us to action your request.
