All new patients can have a health check with our practice nurse.

Register With Us

  • First, check if you live in our area using the map above.
  • Then, fill in the online ‘New Patient Registration’ form.
  • You can sign up at any of our sites.
  • You can be seen at any site and can also pick a favourite surgery.
  • If you are signing up a child under 6 years old, please do it in person at the surgery’s reception. We need to know about their vaccinations.

You can also get registration forms from Reception or print one at home.


It might help if you have one or more of the following:
– Passport
– Birth certificate
– HC2 certificate
– Rough sleepers’ identity badge
– Hostel or Accommodation registration
– Mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
Acceptable photo ID:
– Current passport or identity card
– Resident permit issued by Home office
– Current UK photocard driving licence
– HM Forces identity card
– Police photographic ID card
– University or college photo smartcard
– NHS smart card

Acceptable proof of address:
– Recent utility bill (within 3 months of the current date)
– Local authority tax bill ( valid for current year)
– Bank or building society statement ( within 3 months of a current date)
– Current tenancy agreement
– Court order (within 12 months of the current date)
– UCAS letter (within 12 months of the current date)

Temporary Patient Registrations

If you are sick while away from home, or if you do not have a doctor but need to see one, you can get urgent care from a local GP surgery for 14 days. The GP decides what urgent care you will get.

After 14 days, you will need to sign up as a temporary or permanent patient.

You can be a temporary patient for up to three months. This lets you be on the local surgery’s list but stay a patient at your usual GP.

After three months, you will have to sign up again as a temporary patient or sign up permanently with that surgery.

To be a temporary patient, just contact the local surgery you want to use. Surgeries do not have to take you as a temporary patient, but they must offer urgent care.

You can’t sign up as a temporary patient at a surgery in the town or area where you already have a doctor.

Overseas Visitors

If you visit the UK from another country, you must pay for your healthcare in England.

In 2015, the rules changed about how the NHS charges visitors from other countries. This is so the NHS does not lose money for the care these patients get.

You might be asked for your NHS number when filling in forms online or when you talk to us.

This online tool can help you find your number if you don’t know it.

Please Note

You do not need to know your NHS number to use NHS services. But it can be helpful to have it.

Other ways to get your NHS number

  • Look on any letter from the NHS, like a prescription or appointment letter.
  • Find it in the NHS App after you sign up.
  • Call us and ask for your number.
